This accessible study about the collusion between medical science and the drug industry emphasizes how drug companies market their products by either redefining problems as diseases (like female sexual dysfunction) or redefining a condition to encompass a greater percentage of the population. Moynihan, a health journalist for the New England Journal of Medicine and the Lancet, and Cassels, a Canadian science writer, note, for instance, that eight of the nine specialists who wrote the 2004 federal guideline on high cholesterol, which substantially increased the number of people in that category, have multiple financial ties to drug manufacturers. Physicians now routinely prescribe cholesterol-lowering pills (statins) that may have perilous side effects, when many people could lower their risk of heart attack with less costly and dangerous steps, such as exercise and improved diet. Through aggressive merchandising, funding of medical conferences and expensive perks, drug companies win doctors over to diagnosing these "diseases" and prescribing drugs for them. Unfortunately for these authors, much of this territory has been covered by several books in the past year, most notably Marcia Angell's The Truth About the Drug Companies (due out in paperback from Random House in September). In the opening sections, "Benefits" and "Risks," he provides an excellent overview of the research techniques necessary to evaluate a drug's safety and effectiveness, while in "Cost" and "Information" he covers some of the same territory as Merrill Goozner's The $800 Million Pill and Stephen Ceccoli's Pill Politics regarding the development, marketing, and pricing of new drugs. Avorn's analysis, however, goes further than just criticizing the industry-he puts drug costs in the bigger picture of health resource allocation while pointing out the difficulty that both doctors and patients face in trying to find unbiased information. In the fifth and final section, he suggests more rational public policy. Avorn's coverage is thorough and his writing style engaging and even humorous. While accessible to lay readers, this title would also be an appropriate text for health sciences students. According to Avorn (Medicine/Harvard Medical School), Americans waste billions of dollars on prescription drugs that are overpriced, poorly prescribed, and improperly taken. He draws on his extensive experience as researcher and physician to explore the relationships between benefits, risks, and the economic impact of prescription drugs. While the FDA comes under scrutiny for its failure to properly assess the risks of such medications as the diet drug Redux and the diabetes drug Rezulin, this is not another FDA-bashing book. Avorn's picture is bigger. Through case studies, he examines decisions made by the regulatory agencies, pharmaceutical companies, health policymakers, and physicians that reveal faults in the entire system. After examining how benefits and risks are discovered and measured, he turns to the difficult question of how these are balanced against each other when making decisions about a drug's use. Avorn finds that in the real world, the major source of information about drugs is the promotion departments of pharmaceutical companies. Surrounded by "an almost suffocating plethora of information of very uneven quality," physicians have not been prepared by their medical education to deal with it. He proposes an educational outreach program based in medical schools that would send academic "detail men" to doctors' offices just as drug companies do. They would provide neutral, evidence-based assessment of drug choices so that physicians would know which treatments were more better, safer, and morecost-effective. Avorn points to Canada, Australia, and Great Britain as countries whose drug-assessment procedures we can learn from. His final chapters offer concrete, practical ideas for improving the ways in which drugs are evaluated and pertinent information about them is disseminated. Marked by solid scholarship, measured criticism, and pithy comments: an informative and highly readable study that makes a significant contribution to the ongoing discussion of an important health care issue.

Skloot sketches the similarities between his own mental deterioration due to a brain-ravaging virus and his nonagenarian mother's dementia. This sequel to In the Shadow of Memory (2003) opens shortly after Skloot moves his mother from her home in New York to a long-term care facility near his Oregon residence. For most of her life, Lillian Skloot was a bitter, harsh woman; her son now finds himself navigating an emotionally charged role reversal as he tries to accept the sweet, childlike dependent that she has become. Perhaps the most distinguished aspect of this book, sections of which were previously published in The Best American Science Writing 2003, The Best American Essays 2004 and The Antioch Review, is Skloot's economy in rendering his miserable childhood. Instead of lengthy, purple litanies of youthful horrors, he offers spare sentences that evoke a world. "For years I thought mothers normally bit deep gouges into their own wrists when children spilled a glass of milk," he writes. "I imagined all boys and girls listened while their mothers dialed the phone, asked to talk to the director of the county hospital's 'insane unit,' and asked if there was a room available for a little boy who had disobeyed his mother." The book does, at times, feel disjointed. (At one point, for example, Skloot interrupts an eloquent description of his mother for a seemingly irrelevant recollection of an adolescent romance. Perhaps this disjointedness pushes the reader even further into the author's territory, a place where our minds are not reliable, where we feel dislocated, where we have to come to terms with our own frailty and the frailty of those we love. Deserves a wide audience.

He peppers his chapters with a "subjective memory questionnaire," "right brain" and "left brain" exercises and advanced memory drills that suggest assigning visual images to words and names (e.g., "Weinberger: a bottle of wine falling on a burger"). Employing a chatty writing style that occasionally lapses into infomercial-speak ("But wait, there's more!"), Small manages to intersperse authoritative scientific evidence from leading research universities throughout the text. But much of the information here can be found in a variety of other fitness and lifestyle books, such as relaxation and stress reduction techniques, healthy diet and exercise plans, and examples of the power of positive thinking and keeping mentally active. Small pays scant attention to popular (if unproven) memory-boosting rejuvenators like hormone replacement therapy and ginkgo biloba. Numerous diet books are available for arthritics, but this appears to be one of the best and most comprehensive. Aesoph, a naturopathic physician and medical writer who has written many articles on this topic, covers both traditional and alternative resources. She gives natural dietary information that may reverse and certainly improve many cases of arthritis as well as other auto-immune diseases. Much of her material is not new, but it is refreshingly presented and useful to have compiled in one place. Aesoph discusses "restorative" foods as well as "stressor" food to avoid. She also explains food allergies, outlining simple methods to test for them, and gives medical references for her information. A better purchase than Philip Welsh and Leonardo Bianca's Freedom from Arthritis Through Nutrition (LJ 9/15/92), Aesoph's book is highly recommended for all health collections. Description: This is the fourth edition of a softbound guide to care of the injured patient.
Purpose: A pocket companion with the intent to provide broad scope of coverage is provided.
Audience: Students or trainees on a trauma rotation are an appropriate audience for this work. Editors represent trauma centers throughout the United States with occasional representation from Western Europe. Authors are widely recognized for their expertise.
Features: A comprehensive approach to trauma management is provided in this pocket-sized book. The 50 plus chapters are each approximately 10 pages in length. The book begins with an overview of trauma systems followed by general approaches to the injured patient, care of specific patterns of injury, and then related problems following trauma. Among the problems discussed are the confounding effects of age, drugs and alcohol, thermal injury syndromes, and radiation exposure. A final group of seven chapters covers some of the critical care issues surrounding the management of injury. Occasional use is made of figures and tables, but there are no photographs. References date to within two years of publication and may represent primary or secondary sources. The table of contents provides chapter titles while the detailed index includes separate citations for figures and tables.
Assessment: This manual follows the familiar and successful formula of the larger text, Trauma, 4th edition (McGraw Hill, 2000). Unlike many handbooks patterned after a parent text, however, this manual's presentations and references have been updated by the authors since the larger hardbound reference was published. I recommend the Trauma Manual for conciseness and clarity in presentation and consider this an effective pocket guide for the members of the trauma service.

I was told I had cancer and that I must expect to die soon. Almost eight years later I still do my job and enjoy life. I have not had conventional treatment. Did my cancer simply disappear? Did I do nothing? Far from it. A number of things happened, some by accident, most by design.

Michael Gearin-Tosh is diagnosed with cancer at the age of fifty-four. The doctors urge immediate treatment. He refuses. Intuitively, not on the basis of reason. But as the days pass, Gearin-Tosh falls back on his habits as a scholar of literature. He begins to probe the experts' words and the meaning behind medical phrases. He tries to relate what each doctor says—and does not say—to the doctor's own temperament. And the more questions he asks, the more adamant his refusal to be hurried to treatment.

The delay is a high-risk gamble. He listens to much advice, especially that of three women friends, each with a different point of view, one a doctor. They challenge him. They challenge medical advice. They challenge one another. On no occasion do they speak with one voice. Gearin-Tosh, a tutor at St. Catherine's College, Oxford, was diagnosed with myeloma (cancer of the bone marrow) and told that if he did not begin chemotherapy immediately, he would be dead in less than a year. The recommended treatment, while probably extending his life somewhat, would not cure the condition. A second specialist confirmed the original prognosis, but the author rejected the proposed treatment after a former Oxford pupil consulted a cancer statistician who warned, "If your friend touches chemotherapy, he's a goner." Interwoven with engaging anecdotes from his professional life, Gearin-Tosh details his research into the world of alternative medicine, a journey that led him to Chinese breathing exercises and acupuncture. The treatment that he credits with saving his life he found in A Cancer Therapy, by Max Gerson, a doctor who died in 1958. Based on the daily drinking of freshly made juices and taking several coffee enemas a day, the Gerson Diet also includes a variety of supplements. Despite the time-consuming nature of this restrictive regime, eight years later Gearin-Tosh is alive and pursuing an active professional life. He is careful to point out that while this program is working for him, "each person should explore his own way with their physician." This is a captivating, detailed, and enlightening account of surviving multiple myeloma, a rare and usually incurable cancer. Yet it is much more than the story of a man who questioned the prevailing therapies of conventional medicine. It is the tale of a contemplative person who gave himself the gift of time to reflect over his diagnosis, gather and interpret information, and enlist the support and resources of friends. Rather than rush into debilitating treatment with its incapacitating side effects and questionable benefits, Gearin-Tosh weighed his options and ultimately chose an unconventional, alternative route. Living Proof is also about Gearin-Tosh's relationships with his physicians and researchers and with the friends and colleagues who provided him with support and counsel during his illness. A fellow in English at St. Catherine's College, Oxford, he weaves literary selections from Chekhov to Yeats into his highly personal account, as well as excerpts of correspondence between his physicians and colleagues and relevant citations from the medical literature. It all comes together in an intelligent, beautifully written narrative sprinkled liberally with humor and aplomb. A remarkable first-person account of surviving cancer on one's own terms. Gearin-Tosh (English/Oxford Univ.) received the news that he had incurable multiple myeloma in March 1994 and for the next year kept a journal of his efforts to research his deadly disease, assess his options, and find a benign treatment regimen. This journal, supplemented by numerous letters to and from doctors, colleagues, and friends, form the basis for the first part of Living Proof. He relates how, after consulting a number of doctors in both England and the US, he opts not to undergo the brutal chemotherapy recommended by orthodox medicine. ("If your friend touches chemotherapy, he's a goner" was the warning passed along by someone who had consulted Dr. Ernst Wynder on his behalf.) His research leads him to examine alternative medicine therapies, and he adopts a program utilizing Chinese breathing exercises, acupuncture, regular coffee enemas, and a stringent diet that relies heavily on raw fruits and vegetables supplemented with vitamins and minerals. Gearin-Tosh's fiercely intelligent account ends in 1995, but the second section, which he wrote six years later, sets this apart from the usual cancer-survival narrative. Prefaced by tributes from two physicians, it includes the author's analysis of the situation cancer patients are likely to find themselves in and his advice to them. He argues persuasively against being rushed into treatment by doctors' survival statistics, advocates active involvement in one's own therapy, and urges resistance to the notion that personal temperament or instincts have no place or value in cancer treatment. Also included is a lengthy medical case history, wryly titled "The Caseof the .005% Survivor," written by his doctor and directed to physicians, giving extensive details about his treatment. An afterword directs readers to a Web site where Gearin-Tosh's medical records will be kept up-to-date. Without a hint of bathos or self-pity, and a pleasure to read for its author's intelligence, wit, humanity, and conviction.
